CARE COORDINATION WORKS WITH BRIDGEWATER FALLS TEAM

Bringing care closer to home for patients with multiple conditions and complex health care needs is a key aim of Warrington Health Plus.
A multidisciplinary approach is key to making this happen, and working with the Falls & Rehabilitation team at Bridgewater is one way that the Care Coordination team are achieving this goal.
Often, the patients who are visited by the Care Coordination Nurses may be referred to the Falls team if they suffer from recurring falls and specialised preventative advice is needed to help the patient be safer when out and about or in their own homes.
The Falls & Rehab Team are a multi-disciplinary team offering multi-factorial assessments, which means that specialist professionals look at many aspects of the person's conditions to help correctly assess their needs.
Julia Sutton McGough, Project Manager for Care Coordination explains the work of the falls team: “The Falls & Rehab team receives referrals from a number of colleagues, including GPs and Consultants, Social Care Professionals and Nursing and Residential Care Homes. Warrington Health Plus have referred to this team too, as they are able to carry out complex assessments in relation to falls and osteoporosis risk and at the same time they can also promote falls prevention further throughout Warrington.”
This multidisciplinary approach to care means that the patients receive the most appropriate care assessment by the most suitable professional when they need it.
